A middle aged male with no medical history presented to the ED with left eye pain and vision loss after being hit in the eye. Ocular exam revealed intact extra ocular movements, left periorbital edema and a fixed left pupil, non-reactive to light. Intraocular pressure (IOP) in the left eye was measured at 33 mmHg.

POCUS ocular of the left eye showed a dislocation of the left lens in the posterior pole of the eye.

Pearls: It bears repeating, but if globe rupture is a concern, ocular ultrasound is contraindicated.
