A 30-something year old M presents with central vision loss in the right eye described as looking ‘like a pool of oil.’ He’s only able to see hand motion out of the right eye. Left eye visual acuity is normal. Symptoms for 3 days, no trauma, foreign body sensation, or headache.

POCUS Ocular shows a reflective membrane floating in the vitreous consistent with a retinal detachment.

SONO Pitfall Pearls: Unlike this image, some retinal tears can be very subtle. If the presentation is consistent with pathology, don’t let a seemingly normal POCUS exam dissuade you from pursuing the diagnosis.
